About this Nominee
Jakes Journey Home is a 501c3 nonprofit that assists unhoused veterans and community members into services - to create a lifepath change of success and sustainability. We do the hand holding and liaison work to make sure they don't fall through the cracks of a complicated system. Our unique mobile unit meets folks where they are at, builds trust, assesses the needs and then begins the coordination of services. We have also established a Community Assist Center where folks now have a place to get the hard work completed. Helping our folks work with Courts, Probation, District Attorney's office and Public Defender's office to remove barriers that prevent people from gaining employment and other necessary advancements.
