About this Nominee
Jakes Journey Home is a 501c3 nonprofit that assists unhoused veterans and community members into services - to create a lifepath change of success and sustainability. We do the hand holding and liaison work to make sure they don't fall through the cracks of a complicated system. Our unique mobile unit meets folks where they are at, builds trust, assesses the needs and then begins the coordination of services.
